A RESOLUTION AUTHORIZING THE CITY OF RIVERSIDE TO ENTER INTO A SERVICES AGREEMENT
WITH AND PAY CERTAIN FUNDS TO NORTHLAND HEALTH CARE ACCESS
WHEREAS, Northland Healthcare Access ("Service Provider") is a not-for-profit corporation providing
services, including certain health, emergency and safety services ("Services"), to residents of the Kansas City
region, including the City of Riverside (the "City"); and
WHEREAS, Service Provider desires to continue to provide such Services and requires funding to
continue to provide the Services within the City; and
WHEREAS, the City finds that the provision of the Services is a public purpose and the City has the
capability and desire to assist Service Provider in continuing to provide Services to the public within the City;
and
WHEREAS, the City's Board of Aldermen believes that the City's best interests are promoted and
aided by entering into an agreement with the Service Provider for the provision of the Services; and
WHEREAS, The City and the Service Provider have negotiated and agreed upon an agreement for
the Service and payment for the same;
NOW, THEREFORE, BE IT RESOLVED, by the Board of Aldermen of Riverside, Missouri, as follows:
THAT the City finds that the provision of the Services contributes to the welfare of the City as well as
the economic and social potential of the City and its residents, that the Services are important to the welfare
of the City and that the provision of funds to assist in providing such Services are for a public purpose;
FURTHER THAT the City of Riverside shall pay to the Service Provider the sum of Thirty-five
Thousand and 00/100 Dollars ($35,000.00) to assist it in continuing to provide services to the residents of the
City and the City, pursuant to a Services Agreement, in substantially the form attached as Exhibit A, which is
hereby approved;
FURTHER THAT the Mayor is authorized and directed to execute the Services Agreement in
substantially the same form as set forth in Exhibit A attached hereto. The Mayor and city staff are authorized
and directed to perform all acts and execute any other documents necessary or desirable to effectuate the
intent of this Resolution. /j7-

such Services, and desires to provide Punding Co assist Service Provider iir providirig fc�r the Services for the CiYy's
residents; and
WHEREAS, the City has faund that the provision of the Services contributes tq the welfare of the City as well as

and that the provision of funds ta assist in providing such 5ervices are £or a public purpose.
II�I CONSIDERATIQN af the mutuai covanants and agreements set farth herein, the parties ageee to Che
following:
1. 'Chat Service Provider shall provide the Services for the City pursuanC to the terms uf this Service Agreement
for a term of one ysar commencing on July l, 2014 and ended June 30, 2015.
2. That the City shall pay to Service Provider the Sum of$35,000.
3. 'fhat the Services provided by the Service Provider shalt inelude, but nat be limited to, The activities designed
for Yhe purpose of aiding publi¢ health and safety to tha residents o1'the City, including but not limited Co,
assisting the poor, hungry and hometess residenks of the City with primary health care, and providing
emergency health assistance and relief to residents of the City, in accordance with the policies and procedures
r�fNHCA, thraugh its Serviae Praviders (the "5ervices").
1. Service Provider will provide the City wifh a copy of Service Provider's annual program budget with a brief
sammary of khe accounting controis used. Further expenditure reports including, but not limited to, the
number of Riverside residents served, a detailed explanation of services pravided, and a detailed explanatian
of pragram expenditures is required an an annual basis.
