A RESOLUTION AUTHORIZING, APPROVING AND RATIFYING CHANGE ORDER 3 TO
THE AGREEMENT BY AND BETWEEN SEAL-O-MATIC PAVING COMPANY FOR 2012
STREET MAINTENANCE PROJECT (312-012) RESULTING IN A REVISED CONTRACT
AMOUNT OF $365,673.89 FOR SUCH PURPOSES
WHEREAS, the City issued an Invitation to Bid for the 2012 Street Maintenance Project
which was awarded to Seal-O-Matic Paving Company pursuant to Resolution R-2013-041, and
Change Order 1 to said contract was approved by Resolution R-2013-107; and Change Order 2
to said contract was approved by Resolution R-2013-125; and
WHEREAS, the Board of Aldermen find it in the best interest of the City to formally
authorize, approve and ratify Change Order 3 to the agreement with Seal-O-Matic Paving
Company, resulting in a total revised contract amount not to exceed $365,673.89;
NOW THEREFORE, BE IT RESOLVED BY THE BOARD OF ALDERMEN OF THE
CITY OF RIVERSIDE, MISSOURI, AS FOLLOWS:
THAT Change Order 3 to the Agreement entered into by and between the City and Seal-
O-Matic Paving Company and incorporated herein by reference resulting in a revised contract
amount not to exceed $365,673.89 is hereby authorized, ratified and approved.
FURTHER THAT the Mayor, City Administrator and all other required city officials are
authorized to execute the change order authorized herein together with any and all documents
necessary or incidental to the performance thereof and to take such other actions and/or
execute and deliver for and on behalf of the City all certificates, instruments, agreements and
other documents as may be deemed necessary or convenient to carry out and comply with the
intent of this Resolution.
BE IT REMEMBERED that the above was PASSED AND APPROVED by a majority of
the poard of Aldermen and APPROVED by the Mayor of the City of Riverside, Missouri, this
day of August, 2014.

2012 Street Maintenance Project (312-012)
BACKGROUND: The 2012 Street Maintenance Project consists of:
• New Cul-de-sac in E.H. Young Park
• Reconstruction of Bailey Court
• Mill and overlay of Northwood Rd. from 9 Hwy to the Park Hill entrance
• New trail from 9 Hwy to Northwood Road phase 2
• Montebella subdivision street repairs
• Strathbury street repairs
• Merrimac manhole and street repairs
• R/W stormwater washout repairs on Apache
Change order 3 includes a line item under run for materials not used to close
this project out.
BUDGETARY IMPACT: Approximately $365,673.89 will be expended out of the 2013 Street
Maintenance budget for the construction under this contract.

Total of Previous Changes $52,380.00
Total this Change Order -$1,640.00
Current Contract Total $365,673.89
RECOMMENDATION: Staff recommends approval of the resolution and acceptance of the change order
with Seal-O-Matic Paving Company to close this project out.
