A RESOLUTION APPROVING AN AGREEMENT WITH ETC INSTITUTE
BE IT RESOLVED BY THE BOARD OF ALDERMEN OF THE CITY OF RIVERSIDE,
MISSOURI AS FOLLOWS:
THAT the Board of Aldermen ("Board") approves the Contract for Services and
Scope of Work, in substantially the form attached hereto, with ETC Institute, and
authorizes the Mayor to execute the Agreement on behalf of the City; and
FURTHER THAT the Mayor, the City Administrator, Director of Administration, and
other appropriate City officials are hereby authorized to take any and all actions as may be
deemed necessary or convenient to carry out and comply with the intent of this Resolution
and to execute and deliver for and on behalf of the City all certificates, instruments,
agreements and other documents, as may be necessary or convenient to perform all
matters herein authorized.

Contract for Services and Scope of Work
Between ETC Institute and the City of Riverside, Missouri
ARTICLE I: SCOPE OF SERVICES
1. Overview of Services to Be Performed. ETC Institute will design and administer a
citizen and business survey for the City of Riverside, Missouri. The surveys will be
administered during the spring of 2018. The final report will be completed by April 30,
2018.
2. Maximum fixed fee. The total fee for the project is $12,687. This includes $9,633 to
design and administer the survey, $1,357 for crosstabulations,and $1,697 to complete 25-
50 business surveys. Geocoded mapping is provided for the resident survey at no charge.
3. ETC Institute's responsibilities. The tasks that will be performed by ETC Institute as
part of this agreement include the following:
✓ finalizing the methodology for administering the survey based on input from the City
✓ designing a resident survey instrument that is up to 15 minutes in length(6-7 pages),
and a business survey up to 3 pages in length
✓ selecting a random sample of households to be surveyed
✓ setting up the database
✓ conducting a pretest of the survey instrument
✓ completing 300 surveys by a combination of mail, online and phone(ETC Institute's
costs includes all labor, postage and printing associated with the administration of the
survey). The results of a random sample of 300 completed surveys will have a
precision of at least+1-5%at the 95% level of confidence.
✓ conducting data entry and quality control review for all completed surveys
✓ providing crosstabulations for up to five demographic variables
✓ conducting benchmarking analysis that shows how the results for Riverside compare
to other cities in Kansas and Missouri and Cities across the U.S.

✓ conducting importance-satisfaction analysis to identify the types of improvements that
will have the most impact on satisfaction with city services
✓ completing a final report that will include an executive summary, charts and graphs,
geocoded mapping, benchmarking analysis, importance-satisfaction analysis, tables
showing the results to all questions on the survey, and a copy of the survey instrument
✓ making an on-site presentation of the survey results to the City
4. Responsibilities for the City of Riverside will include the following:
✓ approving the survey instrument
✓ providing a cover letter for the mail version of both business and resident surveys
✓ providing GIS shapefile showing the boundaries of the City
✓ supplying the addresses for households and businesses in the City.
ARTICLE II: PAYMENT FOR SERVICES
1. Invoices will be submitted throughout the duration of the project, for a total project fee of
$12,687.
ARTICLE III: MISCELLANEOUS PROVISIONS
1. Change in Scope. The Scope of Services for this contract shall be subject to modification or
supplement upon the written agreement of the contracting parties.Any such modification in the
Scope of Services shall be incorporated in this agreement by supplemental agreement executed
by the parties.
2. Termination of Contract. This agreement may be terminated by either party upon 14 days
written notice in the event of substantial failure by the other party to perform in accordance with
the terms hereof through no fault of the terminating party. If the contract is terminated by the
City, the City shall reimburse ETC Institute for the full value of any tasks that have been
initiated, up to the total amount of the next scheduled invoice.
3. Rights to Use the Data. ETC Institute has the right to use the data as a component of ETC
Institute's DirectionFinder®benchmarks, but ETC Institute will not release specific results
for the City of Riverside without written approval from the City.

respondents. Survey Owner has received and reviewed a current copy of the ETC Institute Privacy Policy
and understands and acknowledges its terms.
