AN ORDINANCE ESTABLISHING THE RIVERSIDE HEALTHY CITIZEN
INITIATIVE PROGRAM
WHEREAS, the Board of Aldermen in charged with protecting the public health,
safety and general welfare of the citizens of Riverside; and
WHEREAS, in an attempt to carry out this duty, the Board of Aldermen and the
City have created the Riverside Healthy Citizen Initiative Program (the "Program"), the
purpose of which is to allow the citizens of the City to participate in health, wellness and
physical recreation activities.
NOW, THEREFORE, be it Ordained by the Board of Aldermen of the City of
Riverside, Missouri, and follow:
Section 1. The City shall establish the Program to allow residents of the City to
obtain membership in the Platte County Community Center or the North Kansas City
Community Center, with the City contributing 75% of the cost and the resident
participating in the Program paying 25% of the cost.
Section 2. The City shall enter into an agreement with the Platte County
Community Center and the North Kansas City Community Center providing for the
payment of application and membership fees, verification of residency, and collection of
usage data.
Section 3. The City Administration and City Staff are hereby authorized to
perform all the acts and execute any other documents necessary or desirable to effectuate
the intent of this Ordinance.
Section 4. This Ordinance shall take effect immediately.

MEMORANDUM OF UNDERSTANDING
RIVERSIDE HEALTHY CITIZENS INITIATIVE
THIS MEMORANDUM OF UNDERSTANDING~~ the Riverside Healthy Citizen
Initiative (this "Memorandum") is made as of this day of May 2007, by and among
the City of Riverside, Missouri (the "City"), and the Young Men's Christian Association
(the "YMCA") of Greater Kansas City.
WHEREAS, the YMCA and Platte County have entered into a cooperative agreement for
the creation, development, and operation of community centers per Platte County
Commission Order #69-02; and
WHEREAS, this Memorandum is entered into by the above Parties to designate roles and
responsibilities for providing the citizens of the City of Riverside the opportunity to
participate in health, wellness and physical recreation activities via the Riverside Healthy
Citizen Initiative.
NOW, THEREFORE, to memorialize the fundamental understandings of the Parties
hereto in connection with this program for the City; the parties hereby agree as follows:
The City of Riverside will subsidize all participating Riverside residents' monthly
membership fees by 75%, and pay 100% of the joining fees once annually.
• The YMCA shall provide the residents of the City of Riverside rates as follows:
o Resident Contribution: Individual Monthly Rate $12.50
Family Monthly Rate $18.75
o City Contribution: Individual Monthly Rate $37.50
Family Monthly Rate $56.25
• The YMCA agrees to reduce the joining fees to the residents of Riverside to a fee
of $50, which the City will pay once annually. Any additional joining fee will be
the responsibility of the resident.
• The City of Riverside and the YMCA have agreed to a maximum membership
purchase term of six (6) months for the resident.
• The resident will pay membership fees to the YMCA in accordance with the
policies and procedures already set forth by the YMCA.
• The YMCA will give the City of Riverside and participating residents at least 30
days prior notice when there is a change in membership rates.

• The YMCA will send an invoice to the City of Riverside for the City's portion of
the subsidy by the 7~h and the 23'~ of each month and payment is due by the first
working day of the next month.
• The YMCA shall provide to the City an itemization of all monthly membership
fees, joining fees, and monthly pro-rates by each resident.
• The YMCA shall provide to the City reports indicating usage of the program by
the residents.
• The YMCA shall provide to the City all information pertaining to residency
verification within the City; including requiring a copy of a valid driver's license,
social security number, and a copy of a current utility bill invoice with the
resident's name and address.
• This agreement shall terminate one year after the date hereof, unless extended by
mutual written agreement of the Parties
• The YMCA shall send all invoices and related information to the attention of
o City of Riverside

FIRST ADDENDUM TO
MEMORANDUM OF UNDERSTANDING
RIVERSIDE HEALTHY CITIZENS INITIATIVE
THIS FIRST ADDENDUM TO MEMORANDUM OF UNDERSTANDING FOR THE
RNERSID HEALTHY CITIZEN INITIATIVE is made as of this L day of

Yo g Men's Christian Association of Greater Kansas City (the "I'MCA").
WHEREAS, the City and YMCA entered into a Memorandum of Understanding for the
Riverside Healthy Citizen Initiative as of May 14, 2007 (the "Memorandum"); and
WHEREAS, the. City wishes to expand the program to include all full-time employees of
the City and their families, and the YMCA is willing to extend the program to these
employees and families on the same basis as extended to the residents of the City under
the Memorandum.
NOW, THEREFORE, to memorialize the fundamental understandings of the Parties
hereto in connection with this program for the City; the parties hereby agree as follows;
The City of Riverside will subsidize all participating Riverside employees'
monthly membership fees and joining fees, and the YMCA will charge and
collect for fees and provide usage reports to the City, in the same manner as set
forth in the Memorandum.
• The YMCA shall not be required to provide to the City any information
pertaining to residency verification. Upon receiving an application, e YMCA
shall verify employment with the City by contacting v~-~-~
• Upon termination of employment, the City shall notify the YMCA, and the City
subsidy will cease at the end of the current membership term of the.employee.
