A RESOLUTION SELECTING A SERVICE PROVIDER AND BROKER OF RECORD
FOR EMPLOYEE BENEFITS, HEALTH INSURANCE BROKERAGE, AND
CONSULTING SERVICES.
WHEREAS, the City of Riverside, Missouri needs and desires a qualified service
provider for employee benefits, health insurance brokerage, and related consulting services; and
WHEREAS,the City issued a Request for Proposals for such services ("RFP"); and
WHEREAS, several firms responded to the RFP, and three firms were interviewed by
City Staff.
NOW, THEREFORE, BE IT RESOLVED BY THE BOARD OF ALDERMEN OF
THE CITY OF RIVERSIDE, MISSOURI AS FOLLOWS:
THAT Bukaty Companies is hereby selected as the City's service provider and broker of
record for employee benefits,health insurance brokerage, and related consulting services, except
as it may relate to life insurance.
FURTHER THAT the Mayor, the City Administrator, and other appropriate City
officials are hereby authorized to take any and all actions as may be deemed necessary or
convenient to carry out and comply with the intent of this Resolution and to execute and deliver
for and on behalf of the City all certificates, instruments, agreements and other documents, as
may be necessary or convenient to perform all matters herein authorized.
PASSED AND ADOPTED by the Board of Aldermen of the City of Riverside,
