ORDINANCE NO.~~~.~ ~~~
AN ORDINANCE AUTHORIZING THE CITY OF RIVERSIDE TO ENTER INTO A SERVICES AGREEMENT
WITH AND PAY CERTAIN FUNDS TO NORTHLAND HEALTH CLINIC
WHEREAS, Northland Health Clinic ("Service Provider") is anot-for-profit organization providing services,
including certain health, emergency and safety services ("Services"), to residents of the City and the City.
WHEREAS, Service Provider desires to continue to provide such services and requires funding to continue to
provide services within the City.
WHEREAS, the City finds that the provision of the Services is a public purposes and the City has the capability and
desire to assist Service Provider in continuing to provide Services to the public within the City.
NOW, THEREFORE, BE IT ORDAINED, by the Board of Alderman of Riverside, Missouri, as follows:
Section 1. The City finds that the provision of the Services contributes to the welfare of the City as well as the
economic and social potential of the City and its residents, that the Services are important to the welfare of the City and that the
provision of funds to assist in providing such Services are for a public purpose.
Section 2. The City of Riverside shall pay to the Service Provider the sum of Twenty Five Thousand and
00/100 Dollars ($25,000.00) to assist it in continuing to provide services to the residents of the City and the City, pursuant to a
Services Agreement, in substantially the form attached as Exhibit A, which is hereby approved.
Section 3. The Mayor and City Clerk are authorized and directed to execute the Services Agreement with such
changes as such officer shall approve, execution of such document being conclusive proof of such approval. The Mayor, City
Clerk and City Administrator are each authorized and directed to perform all acts and execute any other documents necessary
or desirable to effectuate the intent of this Ordinance. The City Clerk is hereby authorized and directed to ensure that a check
in the amount described in Section 2 is drafted and presented to an authorized official of the Service Provider, upon execution
of the Services Agreement.
Section 4. The sections, paragraphs, clauses, and phrases of the Ordinance are severable and if any portion of
the Ordinance is declared unlawful by the valid judgment, decree, or injunction order of a court of competent jurisdiction, such
ruling shall not affect any of the remaining phrases, clauses, sentences, paragraphs, and sections of the Ordinance and all
provisions of the Ordinance not specifically declared to be unlawful shall remain in full force and effect.
Section 5. This ordinance shall take effect immediately.

't SERVICES AGREEMENT
~~ ~~ THIS SERVICES AGREEMENT entered into on this ~~day of ~'~i , 2002,
by and between the CITY OF RIVERSIDE, MISSOURI ("City") and N RTHLAND HEALTH CARE
ACCESS ("NHCA").
WHEREAS, NHCA is anot-for-profit organization that has contracted with Model Cities Health
Corporation of Kansas City d/b/a Swope Parkway I-Iealth Center ("Service Provider") to provide primary care
health services to residents of the City.
WHEREAS, NHCA, through its Service Provider, desires to continue to provide such primary care
health services and requires funding to continue to provide said services to residents of the City.
WHEREAS, the City has found that the provision of the Services contributes to the welfare of the City
as well as the economic and social potential of the City and its residents, that the Services are important to the
welfare of the City and that the provision of funds to assist in providing such services are for a public purpose.
WHEREAS, the City has the capability and desire to assist NHCA, through its Service Provider, in
continuing to provide primary care health services to the residents of the City.
IN CONSIDERATION of the mutual covenants and agreements set forth herein, the parties agree to
the following:
1. That NHCA, through its Service Provider, shall provide the Services (defined below) for the
City and/or its residents for the next ensuing 12 months pursuant to the terms of this Service Agreement.
2. That the City shall pay to Service Provider the sum of $25,000.00.
3. That the Services provided by NHCA, through its Service Provider, shall include activities
designed for the purpose of aiding public health and safety to the residents of the City, including, but not
limited to, assisting the poor, hungry and homeless residents of the City with primary health care, and
providing emergency health assistance and relief to the residents of the City, in accordance with the policies
and procedures of NHCA, through its Service Provider.
IN WITNESS WHEREOF, the parties have executed this Agreement the day and year first above
